Visit www.aetna.com/docfind/custom/passport.

1. Click on the Aetna Passport to Healthcare®.
2. Enter your “Zip Code” and select the numbers of miles you would like to search
3. Then click, “Search.”
4. Select “Passport to Healthcare® Primary PPO Network” and click “Continue.”
5. Select the type of provider you are searching for.
6. Select a provider from the list, and call to make an appointment, if necessary.

In most cases, the provider will submit the claim for you.

If you are required to pay for services up front, you will need to complete a claim form in order to be reimbursed by the insurance company.
